Fix duplicate key error when submitting reviews

The proposal_peer_reviews table had a unique constraint on (proposal_id, reviewer_id), which caused a DatabaseError when a reviewer submitted comments for multiple sections.

This change modifies the unique constraint to be on (proposal_id, reviewer_id, section_name), allowing multiple comments per review.

The submit_for_review and submit_review functions in backend/api/proposals.py have been updated to align with the new schema.
