feat: Add duplicate knowledge card warning

Adds a modal to the knowledge card creation flow to warn users if a knowledge card with the same outcome, field_context or donor already exists.

The modal includes a link to the existing knowledge card.

Explains in the modal that ad-hoc knowledge cards with specific references and content can be added to offer flexibility.

If the user confirms, they can create the knowledge card.

Also adds a "Duplicate" badge to knowledge cards on the dashboard if they are duplicates.

Includes a delete button on duplicate knowledge cards on the dashboard.
