fix: Remove title column from knowledge card creation

The create_knowledge_card function was attempting to insert a value into the title column of the knowledge_cards table, but this column does not exist in the database schema.

This commit removes the title column from the INSERT statement to align with the database schema and resolve the sqlalchemy.exc.DatabaseError.
