Fix: Concatenate knowledge card title in Chat.jsx

The associated knowledge card display in Chat.jsx was not rendering the card titles correctly because the title property was sometimes undefined. This change fixes the issue by concatenating the card's title, donor_name, outcome_name, and field_context_name to create a descriptive title, similar to how it's displayed in AssociateKnowledgeModal.jsx.

This also includes updates to the tests to ensure they pass with the new logic and to fix existing test failures.
