Commercial invoice enhancements

These are elements often found on CIs but not listed on the KTDDE docs:

  • Add billOfLadingNumber and letterOfCreditNumber
  • Add portOfEntry with UN/LOCODE support
  • Add consignee (separate from buyer)
  • Add destinationCountry field
  • Add freightCost and insuranceCost structures
