Improve web-site structure to reflect CAP as the main way to bring supply chain actors together and to subsequently build extensions.

The process of building extensions comes out of bring different members of a constituent group together and collectively developing how they want to adapt and use UNTP. This process is described as a 'Community Activcation Program'. When you read UNTP - it looks like you should start with an extension rather than to start by bringing together a community. We should refactor the site to better reflect the experience of actually delivering on adoption.
